Mrs. Janet May Kooistra, age 78, of Forkston Township, Dutch Mountain passed away on Friday, December 6, 2013 at the Robert Packer Hospital in Sayre, PA with her loving family at her side.
Janet was born February 25, 1935 a daughter of the late John Raymond Ort and Lillian Marie Maier. She grew up in Hawthorne and graduated from the Hawthorne High School.
